As of 2021, the Toxic decal cannot be obtained or equipped. It probably was a time-limited offer in previous game versions, either from Gift Boxes or as a permanent offer for 
Cash.
However, NPC players can still be seen using the decal in Champions Clash or Seasonal Leagues races.

The only way to obtain this decal would be winning a car equipped with it from a human player in a Pink Slip.
